Chinese Herbal Consultations for your Patients

If you are a Practitioner of Chinese Medicine that is interested in offering your patients customized herbal support, let us help you make that a simple and effective reality!

Here's how it works:

1. Select a date and time to act as a deadline for response. This ensures that Hayley reserves time on her schedule to review your Consultation Questionnaire. This is not an appointment.

2. Once you've selected a deadline, the full Consultation Questionnaire will be displayed. Answer the questions, in as much detail as possible, on behalf of your patient.

3. Payment of $55 will be collected at the bottom of the Consultation Questionnaire page. 

If Hayley Gardner, MSOM DCCM LAc, our herbal medicine expert, has follow-up questions or requires clarification, she will contact you at the email address within 2-3 days.

On or before the deadline, you will receive a detailed information sheet complete with Chinese Medicine pattern(s) present, treatment strategy, customized written herbal formula(s), treatment plan, and detailed formula explanation with references to classical source texts and will answer any questions you might have. 

After receiving the information sheet, you will have the option to have your herbal medicine filled directly through Estuary Herbs compounding pharmacy for shipment to the Practitioner or directly to the patient. If desired, we can input the order for you for a $5 administrative fee. Just email help@estuaryherbs.com to let us know, and we'll ask a few questions to make sure we have all of the necessary details. You may also choose to take this information and fill the formula yourself or have it filled elsewhere.
